Transfer of Medical Records
If you wish to have your records transferred to us from another General Practice please complete this form

When you would like to have your records transferred to another centre or for Medico Legal reasons, please have the requesting party send a signed written request. All persons aged over 14 must sign the authority.
A fee may apply for requests from solicitors or subpoenas, no fee will apply for records transferred to a general practice
